Incident Reporting
The Medical Exposures radiation Unit of the HSE produced Guidelines for reporting patient safety incidents from medical ionising radiation in (2010). These provide guidance on those radiation incidents which must be reported in hospitals and for DEXA scanner practices. Guidelines on Notification of Patient Radiation Incidents in Dentistry were also published in 2010.
The RPII produced separate guidelines on their reporting criteria for radiation incidents involving staff and members of the public (click here). These include exposures to unborn children (where the mother is undergoing a radiological examination) and to patients who are inadvertently exposed in certain situations.
